Job Description

• Reviews referrals and assesses participant’s eligibility for admission in program.
• Completes full nursing assessment and develops medical plan for each participant.
• Assists treatment team in development and implementation of overall treatment plan.
• Actively participates as a treatment team member and performs related tasks.
• Provides medical case management including coordination of services with medical director, medical providers and other related service providers in the program and community.
• Acts as liaison with physicians, pharmacies and families regarding medical issues.
• Performs weekly rounds at program and more frequently as needed.
• Monitors medication administration and trains staff on medication handling, distribution and related policies and procedures.
• Trains and oversees medication coordinators.
• Assists with ensuring compliance with contractual, legal, and regulatory requirements.
• Performs all duties inherent in a supervisory role.
• Performs other duties, as assigned.


Qualifications:  

• Completed Diploma, Associate, or Baccalaureate Degree in Nursing.
• Maintains unrestricted license to practice as a Registered Nurse in the state(s) to which assigned.
• Maintains current BLS (Basic Life Support) or CPR certification, in accordance state regulations.
• A minimum of two-year’s experience as licensed nurse.
• Case management experience and experience in residential environment preferred.
• Communicates effectively and professionally in verbal and written interactions.
• Demonstrates strong organization skills and attention to detail.
• Has reliable transportation and is able to travel to/from local program and community locations throughout work-day and week.
