WHAT YOU CAN EXPECT

• Clinical Best Practice Utilizes evidence-based protocols in developing or revising clinical pathways, standing orders, and policies/procedures. Collaborates with the Education Department, Quality Department, Infection Prevention Department, Emergency Services, as well as Unit Managers and floor staff in identifying staff education/competency needs.
• Continuity of Patient Care Delivery Coordinates management of patient care through multidisciplinary rounding and discharge planning. Provides education to patient and family concerning the treatment goals, plan of care, risk reduction and discharge needs. Acts as a liaison along with Navigators and other disciplines to post-discharge caregivers coordinates transfer of clinical information to promote continuity of care.
• Compliance and Accreditation Coordination Coordinates Compliance with Accrediting, State and Federal Agencies for assigned areas. Inspects the area for compliance with policies, procedures, regulations, and standards and takes action to correct when deficiencies occur. Prepares staff/clinical areas for survey readiness and ensures accurate data submission into appropriate database systems.
• Quality Oversight Suggests policies or practices that should be implemented to support increased quality of care; utilizes evidenced-based protocols in developing policy and procedures.
• Inter- and Intra-Department Communication and Collaboration Maintains inter- and intra- department communications and participates in or chairs assigned meetings or committees or leads special projects. Provides feedback to nursing management regarding appropriate or inappropriate nursing interventions; Collaborates with vascular surgeons, cardiologists, hospital administration and physician offices to identify growth opportunities for the vascular program.

QUALIFICATIONS

• Associate's Degree- Required
• Bachelor's Degree- Preferred
• 2 years Acute care nursing- Required
• 1 year Management/Supervision- Preferred
• Registered Nurse (RN)- Required
• Basic Life Support Program (BLS) - American Heart Association- Required
