Job Description
Summary of Position: The Referral Advocate serves as a bridge between potential patients, their families, and the hospice and Palliative Care Programs. This position is responsible for consulting with patients and families seeking information about EverHeart Hospice services.
The Referral Advocate educates individuals about our services through key contacts with our referral sources in all service areas. This individual will provide additional support to the admissions team and patients, and families. The Referral Advocate will be responsible for continuing to build relationships with organizations and members throughout our service area.

• Provide consultative end-of-life care planning to referral sources and patients, and families, aimed at matching prospects with the best service from our portfolio.
• Visit potential patients and their families at hospitals, residences, or other facilities.
• Serves as the primary referral clinical resource.
• Explain hospice services, care benefits, and program criteria.
• Obtain signatures on the Election of Benefit Statements and Consent for Hospice Statement.
• Assess patient and family readiness for hospice care and address their concerns.
• Assist in patient intake, including obtaining medical history, medical and medication records, and documenting health-related data.
• In coordination with patients and families, prepare and complete pre-admission paperwork according to Agency requirements.
• Coordinate with the Admissions Team to determine the appropriateness and urgency of Hospice and Palliative Care referrals and bring eligible patients onto our services in a timely manner.
• Serve as backup to the other admission consultants and medical secretaries, including office opening and closing procedures.
• General clerical duties to include, but not limited to, utilization of Microsoft Office products, photocopying, faxing, mailing, and filing.
• Collaborate with referral sources to identify strengths and weaknesses regarding EverHeart Hospice admissions.
• Work collaboratively with the Admissions team to facilitate the conversion of referrals.
• Provide back-up support to the Palliative Care Admissions Consultant
• Other duties as assigned.
Qualifications:
Education and Experience: • High School Diploma
• Healthcare Experience Preferred
• Hospice and Palliative Care Preferred
• Healthcare sales and/or marketing experience preferred.
Special Skills and Requirements: • Shall have a valid driver’s license.
• Excellent customer service skills.
• Detail-oriented with excellent communication, documentation, and organizational skills.
• Ability to follow instructions and maintain HIPAA privacy rules and regulations.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to lift 25 lbs. of supplies or equipment. Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.
