Job Description

The Roman Catholic Archdiocese of Washington is home to 140 Parishes and over 350 active and retired priests. We are hiring a Clergy Care Coordinator who will be responsible for providing support to our priests in the areas of health and wellness as well as health care advocacy when navigating medical care. This confidential and trusted position supports the physical well-being of the clergy community while respecting the values of the Catholic church. This role and will report to the Vicar for Clergy. Duties & Responsibilities

  • Support Clergy members on understanding their health and offer suggestions for improvement.
  • Guide clergy to appropriate health resources, support services and specialized medical care to include navigating Medicare and other complex systems.
  • Act as an advocate for clergy with the broader healthcare system, ensuring they receive the necessary care and that their needs are addressed.
  • Facilitate and coordinate care across the health continuum, including hospitalizations, home health services and long-term care facilities.
  • Schedule and coordinate appointments, as needed.
  • Educate clergy on diabetes, hypertension, heart disease and other related medical conditions and provide healthy solutions.
  • Explain and translate medical advice, diagnoses and procedures to priests.
  • Attend doctor appointments as needed to advocate for priest; visit with priests in the hospital.
  • Collaborate with Vicar for Clergy, Priest Coordinator for Pastoral Care for Priest & Human Resources to coordinate care.
  • Promptly inform the Vicar for Clergy regarding illnesses or surgeries.
  • Navigate medication changes and prior authorization (in partnership with Conifer Health Solutions)
  • Complete medication reconciliation and document all intake activity
  • Provide guidance to priests on in- patient and out-patient procedures.
  • Coordinate discharge plans and follow up to include identifying rehabilitation centers, assisted living arrangements and physical therapy appointments.
  • Communicate effectively with priest in a manner that protects their confidentiality and is sensitive to their physical/emotional/spiritual condition.
  • Provide assistance related to Medicare for priests moving from active status to retired status.
  • Acts as a resource for family members and health power of attorneys for retired priests as well as active priests with significant health issues.
  • Other reasonable duties as assigned by the Vicar for Clergy, the Vicar General or the Archbishop.
Qualifications Knowledge, Skills and Abilities
  • Must be a practicing Catholic who supports the teachings of the Catholic Church
  • Outstanding human relation skills and ability to work well with others
  • Knowledge of health services in the DMV
  • Understanding of medical care in accordance with Catholic teachings
  • Demonstrated ability to effectively handle difficult situations.
  • Excellent written and oral communication skills.
  • Ability to handle multiple tasks under time sensitive deadlines.
  • Ability to handle confidential and sensitive information.
  • Ability to use nursing processes to guide patient encounters with compassion and care
Required Education and Experience
  • Bachelor's degree in nursing or registered nurse (RN); active RN license
  • CPR Certification
  • Experience as a health advocate for vulnerable populations, including the elderly, a plus.
  • Knowledge of federal laws including HIPAA, ADA and Medicare
  • Proficiency in suite of Microsoft Office tools: Excel, Word, Outlook, and PowerPoint
  • Experience in community health a plus
  • Spanish skills a plus
Work Environment • This job operates in a professional office environment. This role routinely involves standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.   • Regular full-time employee (40 hours/week); this is an in-office position; Hybrid schedule offered (4 days in the office and one day remote).
  • Reliable transportation required.
  • Travel to Archdiocese parishes and schools required.
