We are seeking a Case Management Assistant to join our team. This Case Management Assistant role will involve supporting transition-of-care and discharge planning activities for ambulatory patients in a high-volume healthcare environment. The ideal Case Management Assistant will have experience with Epic EHR, utilization review support, and administrative coordination, along with a strong ability to work independently while collaborating with interdisciplinary care teams. This Case Management Assistant position supports Pacific Time Zone operations and requires flexibility to work rotating weekends and holidays. The Case Management Assistant will play a key role in ensuring timely, accurate, and well-coordinated patient transitions.
Job Title: Case Management Assistant Location: Mather, CA (100% Remote Supporting Pacific Time Zone) Duration: 26 Weeks (Contract) Job DescriptionThe role provides administrative and coordination support for transition-of-care (TOC) and discharge (DC) planning activities. Responsibilities include assisting Case Managers and Social Workers with post-discharge coordination, utilization review transmission, high-volume fax and voicemail management, and accurate data entry within Epic and related systems. The position prioritizes communication, documentation accuracy, and proactive problem-solving to support timely patient discharge across the continuum of care.
This role requires consistent collaboration with interdisciplinary teams, strong organizational skills, and adherence to all privacy, confidentiality, and regulatory standards. The position operates in a fast-paced environment with shifting priorities and requires availability for rotating weekends and holidays.
Key ResponsibilitiesCollaborate with Case Managers and Social Workers to support TOC and discharge planning
Assist with utilization review transmission and referral coordination
Manage high volumes of faxes, voicemails, and data entry tasks
Coordinate post-discharge referrals, authorizations, and documentation
Monitor TOC progress and escalate barriers impacting timely discharge
Prepare and transmit discharge documentation to post-acute providers
Deliver required patient notifications and discharge-related communications
Maintain accurate documentation within Epic and related systems
Provide clerical and administrative support including scanning, faxing, and reporting
Participate in daily huddles and interdisciplinary coordination meetings
Healthcare Administrative Experience: 1+ year of recent, relevant experience
Epic EHR Experience: 1+ year working within Epic or similar EHR systems
Care Coordination Support: 1 2 years assisting case management or TOC functions
Communication Skills: Strong oral and written communication abilities
Time Management: Proven ability to manage competing priorities in a fast-paced environment
Technical Skills: Intermediate PC and word processing skills
Independence & Judgment: Ability to work with minimal supervision and exercise discretion
